New students shall be interviewed by the Dean of Student Affairs. The following credentials are to be submitted:

1.1 High School Report Card (Form 138)

1.2 Certified True Copy of Birth Certificate or Security Paper (SecPa)

1.3 Certificate of Good Moral Character

1.4 3 copies of 1x1 recent photo

1.5 Marriage Contract for married students

 

2 Transferee's:

2.1 Honorable Dismissal
2.2 Certificate of Good Moral Character
2.3 Certified True Copy of Birth Certificate
2.4 Transcript of Records for Evaluation Purposes
2.5 3 copies of 1x1 recent photos

2.6 Marriage Contract for married students

3 Graduate Students (MA, MBA, MPA, MT):

3.1 Honorable Dismissal
3.2 Transcript of Records for Evaluation
3.3 Special Order (2 photocopies)
3.4 Diploma (2 photocopies)
3.5 Birth Certificate (2 photocopies)
3.6 Marriage Contract (1 photocopy for married female student only)

4 Student may not be re-admitted for the following reasons:

4.1 Gross violation of school policy
4.2 Immorality
4.3 Three (3) failures in a semester
